The 2010 Registration Forms are posted below. The 2010 TRLI Policies and Procedures have been updated and are available below.


2010 Participant Information Each participant needs a Proof of Address and Contact Sheet. Each Family needs to complete this form, listing all children in the household that are registering for TRLI football or cheer this year.

2010 Physician's Certification Form Each participant must be examined by a Physician and certified that they are physically able to play football or be a cheerleader. This form should be completed by your doctor, Signed and Stamped, after the child is examined and turned in to your child's coach before the start of the season. Jersey Shore requires that the Pop Warner Physician's Certification be signed and stamped by your physician.

2010 Participant Contract & Parent Consent The Pop Warner 2009 Participant Contract & Parent Consent form must be filled out for each participant and signed by a parent or guardian.

2010 Volunteer Forms

2010 Volunteer Application Form - For the 7th consecutive season, Pop Warner programs nationwide will be required to annually conduct a background check of: Coaches, Board of Directors members and any other persons, volunteers or hired workers, who provide regular service to the league and/or have repetitive access to, or contact with, players and/or spirit participants.
