2008 Participant Information Each Player needs a Participant Information form. These are preprinted for returning players and that data needs to be verified by the parents at registration. New players need to fill this form out.

2008 Physician's Certification Form Each Player must be examined by a Physician and certified that they are physically able to play football or be a cheerleader. This form should be completed by your doctor, Signed and Stamped, after the child is examined and turned in to your child's coach before the start of the season. Jersey Shore requires that the Pop Warner Physician's Certification be signed and stamped by your physician.

2008 Proof of Address  Each Family needs to complete this form, listing all children in the household that are registering for TRLI football or cheer this year.
